Stroke Clinic: A tool for evaluating Stroke outcomes at King Chulalongkorn Memorial Hospital. (#460)

Background and objectives

After acute Ischemic stroke treatment in the hospital, long-term post-stroke care is essential to prevent recurrent stroke and enhance quality of life. The Stroke clinic is a tool for evaluating stroke outcomes that can help patients and caregivers cope with illness and learn self-care to prevent recurrent stroke and improve Quality of life.

Methods

The Stroke Clinic at KCMH was on every Thursday 08.00-12.00 am.  Stroke patients who were discharged from the hospital after a stroke 90 days were appointed at Stoke Clinic. Stroke nurses and Physicians assessed and planned treatment of patients with Stroke follow-up From Chulalongkorn Stroke Center from January to December 2023: modified Ranking scale (mRS), Barthel’s Index, MOCA, TMSE, and PSDST were the tools.

Result

544 stroke patients had appointments at the stroke clinic. A total of 538 stroke patients (99%) were evaluated. 355 cases (66%) were visited at the stroke clinic. 143 cases (27%) were telephone follow-ups. and were not followed up on 37 cases (7%). Among these  441 cases (82%) were stroke at first times. The mean age of the patients was 68 years.

There were 345 (64.1%) patients who had favorable outcomes (mRS<2) at the 90-day follow-up period. There were recurrence stroke 33 cases (6.1%). 22 cases (4.1%) were dead in 90 days after stroke.  182 cases (33.8%) were cognitive impairment (MOCA <26).

Conclusion.

The stroke clinic is the tool for evaluating stroke outcomes. Stroke outcomes at King Chulalongkorn Memorial Hospital were more favorable at 90 days.
